Meningococcal Disease
Meningococcal disease is a rare but potentially life-threatening bacterial infection caused by Neisseria meningitidis. This bacterium has six main disease-causing strains: A, B, C, W, Y, and X. In the UK, about 10% of people carry the bacteria harmlessly in their nose or throat. Transmission occurs through close contact, including coughing, sneezing, or kissing. Although carriers usually show no symptoms, the bacteria can invade the bloodstream or central nervous system, causing invasive diseases such as septicaemia (blood poisoning) or meningitis (infection of the brain lining). While uncommon, meningococcal disease can progress rapidly and often requires urgent medical care.
Globally, the highest risk occurs in the “meningitis belt” of sub-Saharan Africa, where outbreaks mainly involve serogroup A during the dry season. Travellers to this region, particularly during outbreaks, face increased risk. Large-scale events, such as the Hajj pilgrimage, have also been associated with outbreaks. Attendees must receive a quadrivalent ACWY vaccine, which protects against serogroups A, C, W, and Y, and show proof of vaccination to obtain a visa.
Symptoms
Meningococcal disease often appears as meningitis or septicaemia, both of which can develop rapidly and cause serious complications. Meningitis typically starts with sudden fever, severe headache, neck stiffness, nausea, and vomiting. Septicaemia, on the other hand, may present with fever, chills, confusion, and a distinctive purplish rash.
In severe cases, either condition can progress quickly, leading to organ failure, brain damage, or death without prompt treatment. Recognising symptoms early and seeking immediate medical care is vital, as invasive meningococcal disease constitutes a medical emergency.
Vaccination
Vaccination offers the most effective protection against meningococcal disease, particularly for travellers to high-risk regions or those taking part in activities that increase exposure. The conjugate ACWY vaccine (Menveo® or Nimenrix®) is recommended for travellers visiting areas where outbreaks are common, including the sub-Saharan meningitis belt. Vaccination is also mandatory for anyone attending the Hajj or Umrah.
In addition to the ACWY vaccine, a separate vaccine targeting serogroup B (Bexsero®) is available for people at increased risk in regions where this strain is prevalent.
